Treatment of sudden hearing loss

Sudden hearing loss is a medical emergency that needs to be handled at the earliest. We have specialized services for patients of sudden hearing loss, including administering intratympanic steroid injection (in the ear) by a painless technique to improve hearing. Treatment of sudden hearing loss must be done as early as possible because the final results become drastically poorer with time. The sooner the treatment is started the better is the final result. We also do intratympanic treatment for difficult cases of Meniere’s disease and Tumarkin’s crisis.

Audiometry tests:

Tests that help us measure how well a person can hear sound at different frequencies and loudness.

  • Pure Tone Audiometry: this is a standard test to measure the ability to hear sound at different loudness(pitch)and frequencies. Headphones deliver sound to test the air conduction and bone conduction pathways of sound transmission.
  • Speech Audiometry: This is a specialized test to assess the type and degree of hearing loss using speech cues (SD ,SRT/SIS)
  • UCL/LDL and MCL: these are the tests that help us determine the loudness levels which are most comfortable and uncomfortable. This is important for hearing aid fitting and sound therapy.
  • Dynamic Range: allows us to understand the best fitting range for hearing aids.

Special Hearing Tests

these tests help to discriminate between cochlear (inner ear) disorders and retro-cochlear (hearing nerve) disorders.

Cochlear pathology

it affects the inner ear which is responsible for converting sound waves into signals that the brain can interpret as sound. This type of hearing loss is seen in noise induced hearing loss and presbycusis (age related hearing loss). This is also influenced by how long the individual has presented with hearing loss and the severity of loss. Hearing Aids are often an appropriate treatment option for those with cochlear pathologies.

Retro-cochlear Pathology

This arises beyond cochlea and affects the vestibulo-cochlear nerve (VIIIth cranial nerve) or central auditory system. This can arise due to infections, demyelinating diseases like multiple sclerosis, auditory neuropathy or acoustic neuroma.

Special hearing tests include

  • SISI (short increment sensitivity index): Measures ability to recognize increases in loudness during a series of pure tone presentations
  • ABLB (Alternate Binaural Loudness Balance): This test measures the difference in how loud sounds are perceived in each ear. It helps in diagnosing recruitment (sudden loudness increase) and reduced dynamic range.
  • Tone Decay: Test that measures auditory fatigue and can help identify the cause of hearing loss

Middle Ear Function Tests

Tympanometry

This is a test to measure compliance of the middle ear to identify middle ear infections, perforation in eardrum and fluid in middle ear.

Stapedial Muscle Relaxometry

Check for muscle contraction in response to a loud sound.

Eustachian tube function

This test helps in assessing how well the eustachian tube equalizes pressure in the middle ear. The Eustachian tube allows the eardrum to vibrate properly and transmit sound. It serves as a passage for middle ear ventilation and mucus to drain.

Inner Ear Function Test

OAE (Oto-Acoustic Emissions): It helps in detecting outer hair cell function. It is useful in early detection of hearing loss.

Types of hearing aids

BTE (Behind the ear)

These are suitable for mild to profound hearing loss. The device sits behind the ear, with tubing connecting to an ear mould inside the ear

RIC (Receiver in the cancal)

This is a newer, smaller hearing aid. RIC delivers sound through a thin wire. It is comfortable and less noticeable compared to BTE. It is specially useful for people with high frequency hearing loss. The “open fit “option offers a more natural sound experience.

ITC (IN the canal)

These fit partially in the ear canal and are less visible.

CIC (completely in the canal)

These hearing aids fit deeply inside the ear canal and are very discreet.

IIC (Invisible in the canal)

These are custom made to fit completely inside the ear canal, making them virtually invisible.
